Clinical Pharmacist: Patient-Centered Care & Dosing Expert
American Geriatrics Society
The American Geriatrics Society is seeking a Staff Pharmacist to provide patient-centered pharmaceutical care alongside distributive and drug-control duties. Candidate will handle antibiotic and anticoagulant dosing, initial warfarin dosing, renal dosing, and IV-to-PO conversions, advising the medical team on interactions and contraindications. You will participate in daily rounds, educate patients and nurses, ensure accurate medication profiles, and support discharge counseling, while complying #J-18808-Ljbffr American Geriatrics Society
