Description
  • Assess patients in both inpatient and outpatient settings, creating tailored treatment plans
  • Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
  • Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
  • Inform patients and their families about therapeutic objectives, discharge planning, and at-home exercises
  • Maintain meticulous documentation of evaluations, treatments, and progress in compliance with facility and regulatory standards
  • Engage in care team meetings and interdisciplinary discussions to ensure seamless patient transitions
Education
  • Doctorate in Physical Therapy (DPT) or a relatable degree from an accredited institution
Certifications
  • Current state licensure as a Physical Therapist
  • CPR certification preferred
  • Additional certifications (e.g., in neurology or geriatrics) are appreciated but not mandatory
Skills
  • A background in physical therapy is strongly preferred
  • Expertise in treating complex medical conditions and mobility challenges
