- Assess patients in both inpatient and outpatient settings, creating tailored treatment plans
- Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
- Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
- Inform patients and their families about therapeutic objectives, discharge planning, and at-home exercises
- Maintain meticulous documentation of evaluations, treatments, and progress in compliance with facility and regulatory standards
- Engage in care team meetings and interdisciplinary discussions to ensure seamless patient transitions
- Doctorate in Physical Therapy (DPT) or a relatable degree from an accredited institution
- Current state licensure as a Physical Therapist
- CPR certification preferred
- Additional certifications (e.g., in neurology or geriatrics) are appreciated but not mandatory
- A background in physical therapy is strongly preferred
- Expertise in treating complex medical conditions and mobility challenges
- Exceptional communication and teamwork abilities
- Familiarity with digital documentation and clinical charting systems
