About the Position

Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year License: RN License required Certifications: Certified in Home Care Must-Have: - 1 year Certified Home Care experience - 1 year Case Management experience - Ability to travel to cases as assigned - Weekend rotations are once every 6 weeks Description: As a Home Health RN, you will be responsible for assessing patient conditions. You will implement care plans and provide ongoing education and support to patients and their families. Patient care will be delivered within organizational philosophy, policy, and standards of community health practice. You will also be required to perform admission processes and case management responsibilities. This will be in accordance with relevant policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communication will be utilized.
