Key Responsibilities\n

• Monitor patients, particularly those adjusting to dialysis and managing chronic conditions, to facilitate their psychosocial adjustment and optimize their mental wellbeing.

• Provide supportive counseling tailored to the individual needs of patients within the scope of clinical training and licensure, empowering them to engage with their treatment processes.

• Assist patients who are exploring transplant options through educational and goal-directed counseling, preparing them for necessary lifestyle changes and emotional adjustments.

• Collaborate with patients and healthcare teams to facilitate referrals to essential community resources, including home health services and vocational rehabilitation, to ensure holistic care and improved treatment outcomes.

• Maintain updated knowledge of local vocational and educational rehabilitation programs to aid patients in accessing these services for employment and educational pursuits.

• Review and discuss advance directives with patients, helping them articulate their healthcare wishes with families and healthcare providers when necessary.
