Job Title Physical Therapist Location Seminole, Florida Schedule / Job Type Contract | 13-Week Assignment | Skilled Nursing Facility (SNF) Key Responsibilities Evaluateand treat patients in a skilled nursing facility setting to restore and improve physical function Develop and implement individualized physical therapy treatment plans based on patient needs and goals Monitor and document patient progress, adjusting treatment plans as clinically appropriate Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning Educate patients and their families on therapeutic exercises and home safety strategies Required Qualifications Active Physical Therapist (PT) license in the state of Florida Graduate of an accredited Physical Therapy program Experience working in a skilled nursing facility (SNF) setting preferred Current BLS/CPR certification Preferred Experience / Skills Prior experience in a SNF or long-term care environment Familiarity with MDS documentation and Medicare compliance in a SNF setting Why Join Us Opportunity to make a meaningful impact on patient recovery and quality of life in a supportive care environment Gain valuable contract experience in a skilled nursing setting with a defined 13-week engagement
