Salary: $19 hr

Schedule: Monday - Friday 8am - 5pm

Job Summary:

The Case Manager provides direct case management and resource navigation services to individuals and families seeking assistance through MesaCAN – Client Emergency Services. This position conducts client intake and eligibility screening, coordinates available assistance and referrals, maintains accurate case documentation, and connects clients with community resources and benefits. The Case Manager works in accordance with program guidelines, funding requirements, agency policies, and applicable regulatory requirements while providing professional, respectful, and client-centered service.

Responsibilities:

1. Conduct client intake, screening, and needs assessments to identify potential eligibility for MesaCAN programs, assistance, and other available services.
2. Review client inquiries and referrals, communicate with clients regarding program requirements, and identify documentation necessary to complete eligibility and service determinations.
3. Conduct initial and ongoing eligibility assessments and re-assessments for available programs and services in accordance with established program guidelines, funding requirements, and documentation standards.
4. Verify income, household, residency, and other required eligibility information and obtain supporting documentation in accordance with applicable program, funding, grant, and agency requirements.
5. Process approved client assistance and benefits in accordance with established program procedures, authorization requirements, funding limitations, and internal controls.
6. Create, maintain, and update complete and accurate client records, including demographic information, eligibility documentation, case notes, services provided, referrals, and required supporting documentation.
7. Provide ongoing case management and follow-up based on client needs, program eligibility, available resources, and established service plans.
8. Assist clients and families with identifying and accessing community resources, benefits, assistance programs, and other supportive services that may help address identified needs and goals.
9. Coordinate referrals and services with community partners, service providers, government agencies, and other A New Leaf programs as appropriate.
10. Schedule and participate in client, family, case management, and other service coordination meetings as appropriate, and complete required case or staffing documentation.
11. Provide clients with clear and accurate information regarding available services, eligibility requirements, required documentation, program limitations, referrals, and next steps.
12. Maintain appropriate professional boundaries with clients and community partners and avoid actual or perceived conflicts of interest. Disclose potential conflicts and refer matters involving employees, relatives, personal relationships, or other potential conflicts to a supervisor in accordance with agency requirements.
13. Follow established approval processes and promptly elevate eligibility questions, unusual circumstances, requested exceptions, or situations outside established program guidelines to the appropriate supervisor before committing or providing assistance.
14. Protect confidential and sensitive client information and access, use, disclose, and maintain records only as authorized and necessary to perform assigned job responsibilities.
15. Complete case documentation, data entry, reports, and other required records accurately, timely, and thoroughly in accordance with agency, program, funding, grant, contract, and applicable regulatory requirements.
16. Monitor assigned cases and follow up with clients, families, service providers, and community partners as necessary to support service coordination and case completion.
17. Maintain current knowledge of available community resources, eligibility programs, referral options, and MesaCAN services to support effective resource navigation.
18. Communicate professionally and respectfully with clients, families, coworkers, community partners, and other stakeholders, including individuals experiencing crisis or other challenging circumstances.
19. Demonstrate cultural awareness and responsiveness when serving individuals and families from diverse backgrounds and communities.
20. Be available and responsive to clients, families, community partners, and other stakeholders during scheduled business hours.
21. Participate in program staff meetings, training, case consultation, quality improvement activities, and other meetings as assigned.
22. Support community outreach and program activities intended to increase awareness of MesaCAN services and connect potentially eligible community members with available resources.
23. Schedule client appointments and obtain required authorizations, as applicable to program and client needs.
24. Follow established safety, incident reporting, mandated reporting, confidentiality, fiscal accountability, and client service procedures and promptly report concerns through appropriate channels.
25. Perform other reasonably related duties consistent with the position's responsibilities and organizational business needs.

Qualifications:

Education:

• High school diploma or GED required.
• Associate degree in Human Services, Social Services, Behavioral Health, Community Services, or a related field preferred.

Experience:

• Two (2) years of related experience in case management, human services, social services, community services, benefits or eligibility programs, nonprofit services, or a related field required.
• Experience conducting client intake, eligibility screening, resource navigation, or service coordination preferred.
• Knowledge of community resources and public or community-based assistance programs preferred.
• Experience working with individuals and families experiencing barriers to stability or accessing community-based assistance preferred.
• Bilingual verbal communication skills in English and Spanish preferred.

Working Conditions/Physical Requirements:

• Work is performed onsite in an office and client-service environment, with local travel as needed to support program activities.
• Ability to sit for extended periods and regularly use a computer, telephone, and other standard office equipment.
• May occasionally lift or move materials, supplies, or equipment weighing up to 25 pounds.
• May interact with individuals and families experiencing crisis, financial hardship, housing instability, emotional distress, or other challenging circumstances.
• Must be able to maintain confidentiality, professionalism, appropriate boundaries, and composure in all interactions.
• Ability to manage multiple tasks, maintain accurate documentation, and meet established deadlines.
