Part Time / Weekend Physical Therapist Case Manager: Home Health

Overview Part Time/Weekend Physical Therapist Case Manager in the Solano coverage area. In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. Responsibilities Perform an evaluation in the home and develop a care plan and work with Physical Therapy Assistants (PTAs) to follow the care plan. Help patients with mobility, movement, and regain independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a high level of care. Evaluate and report patients’ progress to the MD. Qualifications Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources Understanding of development, behavior, culture in health care Knowledge of state and federal homecare regulatory guidelines (preferred) Home health care experience (highly preferred) Experience with high acuity patients (a plus) Experience working with an interdisciplinary team (highly desired) Case Management fundamentals (highly desired) Self-directed, empathetic, critical thinker, outstanding interpersonal skills Leadership and organizational skills Compensation and Benefits $120,000 - $135,000 per year. Wages for Physical Therapist, Level 1: $120,000.00 - $135,000.00 plus $10,000 annual bonus, paid quarterly. As a practice, most PT Case Managers are hired as PTIs; promotion to PTII with higher case load and autonomy includes a minimum salary of $135,000 plus a $20,000 quarterly bonus.
