Responsibilities

• Conduct initial and ongoing physical therapy evaluations for residents in a skilled nursing setting.
• Develop and implement individualized treatment plans aimed at improving mobility, strength, balance, gait, and functional independence.
• Deliver therapeutic interventions including therapeutic exercise, gait training, ROM work, and appropriate modalities.
• Monitor resident progress, document outcomes, and adjust plans in collaboration with the interdisciplinary team.
• Support discharge planning through collaboration with nursing, therapy colleagues, and other care providers; communicate progress and goals to residents and families as appropriate.
• Ensure patient safety, adherence to infection control protocols, accurate documentation in the electronic health record, and compliance with facility policies.

Requirements

• Active, unrestricted physical therapy license (state-specific) to practice in the United States; ability to obtain Maine licensure if not already licensed.
• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• CPR/BLS certification or equivalent (as required for clinical practice).
• Relevant experience in geriatric rehabilitation or skilled nursing settings preferred; strong comfort with documentation in electronic health records.
• Ability to work in a skilled nursing facility environment and to meet the 13‑week travel assignment requirements.
• Effective communication and collaboration skills for working with an interdisciplinary team and resident’s families.
