Role Overview Seeking a licensed Role Overview Seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in private residences and senior living communities. Key Responsibilities Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions to enhance mobility, independence, and quality of life. Document patient progress accurately within electronic medical records, ensuring compliance with standards. Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Utilize telehealth and virtual care options to extend service accessibility and flexibility. Qualifications & Skills Valid Physical Therapy license in practice state(s) or eligibility to obtain one. Degree from an accredited PT program. Proficiency with basic computer use and EMR systems. Current CPR certification. Physical & Additional Requirements Ability to exert medium physical effort, lift up to 50 lbs, and assist with patient transfers. Repositioning, standing, walking, climbing stairs, and performing therapeutic activities. Driving to patient sites and wearing PPE when necessary.
