As a Wound Care Nurse Practitioner, you'll provide on-site care throughout assisted living communities, memory care settings, skilled nursing facilities, and long-term care communities across the Twin Cities Metro. You'll manage complex wounds, support chronic disease management, and help patients receive specialized care without unnecessary barriers.

What You'll Be Doing

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help reduce avoidable hospitalizations and improve patient outcomes.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
