The RN Home Services Case Manager leads patient‐centered care in the home by developing individualized plans that promote independence, safety, and quality of life. This role serves as the primary nurse from admission through discharge, completes OASIS assessments, and coordinates care across caregivers, providers, and community resources to ensure seamless, high‐quality outcomes. We're looking for a dynamic and compassionate RN Case Manager to join our Home Health team. In this vital role, you'll lead the coordination and delivery of exceptional patient care within the home setting—serving as a central figure on our interdisciplinary team.
You'll take ownership of the entire patient care journey, from admission through discharge, ensuring seamless communication, high-quality outcomes, and efficient resource use. Your leadership will guide planning, education, and collaboration across disciplines, while fostering strong relationships with patients, families, and payors.

Current Registered Nurse (RN) license in state of practice. Bachelor of Science in nursing (BSN) from an accredited institution (degree verification required).
Basic Life Support Certification (BLS) for Healthcare Providers.
Unit specific certification as required (i.e., Demonstrated clinical nursing experience.
Demonstrated knowledge of home health or hospice.
Ability to communicate effectively both verbally and in writing.
Case Management Certification
One (1) year experience working in home health or hospice.
Three (3) years of clinical nursing experience.
Previous experience in case management, utilization review, and/or discharge planning.

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
Will also bend to retrieve, lift, and carry supplies and equipment. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
