Position Title: Licensed Practical Nurse (LPN) Long Term Care (LTC)

Location: Milbank, SD 57252

Duration: 13 weeks (Possible Extension)

Work Schedule: Variable Day Shift | 12-Hour Shifts

Shift Timings: Day: 6:00 AM 6:30 PM / Night: 6:00 PM 6:30 AM

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to long-term care residents.
• Administer medications and treatments as prescribed by physicians.
• Monitor residents' conditions and promptly report any changes.
• Maintain accurate documentation using the PCC charting system.
• Assist in developing and updating individualized care plans.
• Collaborate with physicians and the interdisciplinary care team.
• Ensure compliance with facility policies, infection control standards, and state/federal regulations.
• Promote a safe, compassionate, and resident-centered care environment.

To be considered for this position, you should have:

• Active South Dakota LPN License (or Compact License, if applicable).
• Minimum of 1 year of Long Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred.
• Current BLS Certification.
• Strong clinical assessment, medication administration, and documentation skills.
• Ability to work 12-hour day shifts, weekends, and holidays as scheduled.
