Registered Nurse – Case Management

Schedule: 7:00 AM - 7:00 PM

Assignment Details:

• Contract Weeks: 5
• Guaranteed Hours: 40.00

Requirements:

• Bachelor's Degree in Nursing (BSN): Preferred
• Registered Nurse (RN) licensure in the state of practice: Required
• Case management certification: Preferred
• At least two years recent experience in field
• Current BLS certification

Roles & Responsibilities:

• Provide excellent patient care by collaborating with the multidisciplinary team to develop, implement, and monitor care plans.
• Act as a liaison between patients, families, and healthcare providers to optimize healthcare outcomes.
• Lead coordination of patient care by monitoring the appropriateness and timeliness of services.
• Address the needs of complex patients, including those with chronic conditions and frequent emergency department utilization.
• Ensure continuity of care through efficient discharge planning and transfers as needed.
• Collect data for regulatory and accreditation compliance.
