Hospice RN Case Manager - Patient-Centric Care Leader

Hospice of Virginia in Richmond, VA is seeking an RN Case Manager to support patients and families facing end of life. You will assess needs, plan and coordinate home-based nursing care, and collaborate with the interdisciplinary team to ensure compassionate, quality care.
Shift Information: Monday–Friday 8a–5p with rotating on call. Comprehensive benefits offered including medical, dental, vision, 401(k) with match, PTO, tuition reimbursement and holidays.
