Registered Nurse (RN) - Senior Living / Long-Term Care

Location: Salem, OR (Travel throughout Marion, Clackamas, Benton, Linn, and Polk Counties)
Schedule: Monday-Friday | 8:00 AM - 5:00 PM
Duration: 6-12 Month Contract
Setting: Senior Living / Long-Term Care

Position Overview

We are seeking a Registered Nurse (RN) to support care coordination and clinical operations within Senior Living and Long-Term Care communities. This role partners closely with providers, facility staff, residents, and families to manage patient care, ensure accurate documentation, support quality initiatives, and coordinate services for complex and chronic health conditions.

Key Responsibilities

• Coordinate care with providers, facility staff, and interdisciplinary care teams.
• Ensure complete and accurate clinical documentation, coding, and medication reconciliation.
• Participate in nursing assessments, rounds, family conferences, and care planning meetings.
• Monitor and respond to changes in patient condition and coordinate appropriate interventions.
• Support management of acute illnesses, chronic conditions, and sub-acute rehabilitation episodes.
• Assist with therapy coordination, benefit verification, and quality compliance initiatives.
• Conduct staff education and participate in facility-based meetings.

Required Qualifications

• Active, unrestricted RN license.
• Associate Degree or higher in Nursing.
• Current BLS Certification.
• Ability to perform physical assessments and meet job-related physical requirements.
• Reliable transportation and ability to travel between facilities.
• Ability to pass required drug screening and compliance requirements.

Preferred Qualifications

• 1+ year of RN experience in Long-Term Care, Skilled Nursing, Senior Living, or Geriatrics.
• Experience managing patients with complex medical, behavioral, social, or functional needs.
• Knowledge of chronic disease management and geriatric care.
• Proficiency with EMR/EHR documentation systems.
• Strong communication, organization, and care coordination skills.
