Job Overview This is a 3rd Shift (Overnight) position. The Care Manager/'Designated Care Manager' is responsible for providing high‑quality care and services to a consistent group of residents and their families in our assisted living/long‑term care and reminiscence neighborhoods. The role includes hands‑on care, physical and emotional support as outlined in each resident's Individualized Service Plan (ISP), and maintaining a safe and comfortable home‑like environment. The Care Manager demonstrates Sunrise's mission: “to champion quality of life for all seniors,” in accordance with federal, state, and local laws and regulations and Sunrise standards, policies, and procedures. Responsibilities Resident Care: Review, read, notate, and initial Daily Log to document pertinent resident information. Participate in the development of ISPs and monthly updates. Review designated assignments and maintain awareness of resident locations, physically checking on them throughout the shift. Observe, report, and document symptoms and changes such as skin, behavior, alertness, weight, diet, and activity participation. Attend daily Cross Over meetings and notify supervisor and resident care director of increased care needs. Respond to security system and resident call bells promptly, reset call bells as needed. Greet guests, family members, residents, and team members; answer phone calls courteously and professionally. Assist with continence management and disposal of products to ensure sanitation. Communicate with families as a resource and when needed. Help residents maintain independence, physical safety, and dignity in accordance with Resident Rights and Sunrise Principles of Service. Participate, lead, and assist residents with activities of daily living (ADLs) and IEA activities indicated on the ISP. Engage residents in life‑skills and enrichment activities throughout the day in reminiscence. Maintain a warm, personal, positive atmosphere and adhere to safety regulations. Risk Management and General Safety: Ensure community complies with national/provincial occupational health and safety requirements. Practice safety procedures including PPE, fire extinguishers, SDS, lock‑out/tag‑out, and report all accidents, incidents, unsafe conditions, and hazardous equipment immediately. Safely store and monitor oxygen tanks and handle blood‑borne pathogens and hazardous materials appropriately. Housekeeping and Laundry Services: Maintain and clean resident rooms (beds, trash, clothes, spot‑sweeping, mopping). Wash resident laundry, dining room linens, and napkins; complete assigned housekeeping tasks. Maintain common areas clean and tidy at all times. Dining Service: Serve meals, promote pleasant dining experiences, and assist with dining room setup and clean‑up. Participate in pre‑meal meetings; follow resident diets per ISP and dietary board. Document changes in dining habits in daily log and address dining needs for residents and guests. Provide room service delivery as needed; practice safe food handling compliance. Specific Responsibilities for the Reminiscence Neighborhood Care Manager: IEA residents in life‑skills and enriching activities, and in afternoon social. Incorporate ‘Creating Pleasant Days’ into daily routine. Integrate individual residents’ life skills into daily routine. Blend multi‑sensory experiences into the resident’s day. Participate in monthly letters home and letter writing with residents and families. Training and Team Success: Participate as a team member toward team goals and daily interactions in alignment with Team Member Credo. Contribute to engagement programs, attend training modules, and comply with infection‑control techniques. Maintain compliance with required training and regulatory requirements. Perform other duties as assigned. Core Competencies Remain calm and composed in difficult situations. Handle multiple priorities efficiently. Demonstrate effective written and verbal communication. Apply organizational and time‑management skills. Show good judgment, problem‑solving, and decision‑making abilities. Experience and Qualifications High School diploma or GED (may be required). CPR Certificate and First Aid per state/provincial regulations. Minimum age 18 years. Previous experience working with seniors preferred. Desire to serve and care for seniors. Ability to make choices and decisions and act in the resident’s best interest.
