Case Manager - Acute Care

Location: Eureka, California

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift, 5x8-hour schedule (08:00 AM - 04:30 PM). Required to work every other weekend.

Job Description: Seeking an experienced Case Manager with a strong background in acute care. The ideal candidate will be responsible for ensuring effective patient care coordination, managing admission and discharge planning, and overseeing case management processes in compliance with regulations.

Responsibilities:

• Conduct assessments and develop care plans for patients.
• Coordinate with healthcare providers and teams to deliver comprehensive patient care.
• Manage and facilitate patient admissions and discharges.
• Assist with appeals and denials, benefits eligibility, and prior authorizations.
• Ensure compliance with CMS regulations and HIPAA guidelines.
• Provide case management support for various patient demographics including adolescents, adults, and geriatrics.

Required Experience / Certifications:

• Minimum of 2 years of acute care experience.
• Proven experience in case management, including familiarity with HMOs, IPAs, advance directives, and protective services processes.
• Knowledge of CMS regulations, discharge planning, and needs assessment.
• Ability to float between like units based on department needs.
• Certification in case management is preferred.
