Position Summary

We are seeking a dedicated Registered Nurse (RN) for a home health assignment. The successful candidate will provide patient care through home visits, ensuring high-quality nursing services to patients in their homes.

Contract Details

• Contract Length: 13 weeks
• Schedule: Monday to Friday, 8:00 AM - 5:00 PM
• Weekend Rotation: None
• On-Call Requirement: None

The RN will cover the following regions:

• Fitchburg, MA
• Leominster, MA
• Lunenburg, MA
• Princeton, MA
• Westminster, MA
• Sterling, MA
• Clinton, MA
• Holden, MA
• Boylston, MA
• West Boylston, MA

Job Responsibilities

The Registered Nurse will be responsible for the following:

• Conducting home visits to provide nursing care to patients.
• Maintaining a weekly productivity expectation of 30 points.
• Documenting visit types, travel, and other activities as required.
• Attending periodic office meetings.
• Maintaining compliance with all documentation requirements, ensuring timely completion within 24 hours post-appointment.
• Collaborating with other healthcare professionals to ensure comprehensive care.

Patient Demographics and Care Requirements

Clinicians will manage an average daily caseload of up to 58 patients, which may include:

• Start of Care
• Wound Care
• Follow-up Visits
• Other adult home health services

Qualifications

• Minimum of one year of recent adult home health or hospice experience.
• Must possess an active RN license.
• Required certifications: CPR and BLS.
• Proficiency in Homecare Homebase (HCHB) within the last six months.
• Comfort with traveling up to 75 miles for patient visits.
• Reliable transportation and valid driver's license with necessary auto insurance.

Work Environment

The position involves a combination of driving to patient homes and attending weekly meetings at the designated clinic. Clinicians are expected to adapt and respond to varying patient needs and schedules while maintaining high standards of care.

Additional Information

Please note that there are no guaranteed hours associated with this assignment. Clinicians will be expected to accept patients based on their schedule, regardless of travel distance to ensure continuity of care.
