Job Summary:
Occupational Therapist (OT) specializing in Home Health in Clearlake, California. This role involves planning and conducting segments of treatment programs aimed at restoring and enhancing patients' physical, social, and mental functions. Candidates are required to bring their license on the first day of employment. The position offers a Monday through Friday day shift with no call or weekend requirements.

Job Details:

• Location: Clearlake, California
• Schedule: Day Shift, 8:00 AM to 4:00 PM, Monday through Friday, 40 hours per week
• Required Qualifications: Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) obtained license on or after January 2007, or foreign equivalent; valid Occupational Therapy licensure in all applicable states of practice; valid Driver's License; at least one year of home health experience as an Occupational Therapist; CPR or BLS certification from an approved vendor
• Preferred Qualifications: Experience with electronic medical records (EMR) systems such as HCHB, proficiency in clinical techniques, and development of individualized treatment plans
• Additional Information: The role involves completing evaluations, providing therapy interventions, developing discharge plans, supervising assigned assistants, and ensuring timely clinical documentation. All required vaccinations must be received. Mileage reimbursement is provided according to government mileage rates. No call and no weekend work.
