Home Care Physical Therapist

University Home Care Inc is seeking a compassionate and skilled Home Care Physical Therapist to join our team in Livonia, MI. In this role, you will provide high-quality physical therapy services to patients in their homes, helping them improve mobility, manage pain, and enhance their overall quality of life. Responsibilities Conduct in-home physical therapy evaluations and assessments Develop and implement individualized treatment plans based on patient needs Provide therapeutic interventions to improve patient mobility and function Monitor patient progress and adjust treatment plans accordingly Educate patients and their families on exercises and care techniques Maintain accurate and timely documentation of patient care Collaborate with healthcare providers to ensure continuity of care Requirements Valid Physical Therapist license in the state of Michigan Experience in home health or outpatient physical therapy preferred Strong interpersonal and communication skills Ability to work independently and manage a flexible schedule Reliable transportation for travel to patient homes in the Southeast MI Region Commitment to providing compassionate patient-centered care Benefits Competitive hourly rate: $70.00 - $80.00 per visit, paid bi-weekly Flexible scheduling opportunities Supportive and collaborative work environment Opportunities for professional growth and continuing education About the Company University Home Care Inc is dedicated to delivering exceptional home health services to our community.
