Speech Language Pathologist (SLP) - Long Term Care

Schedule: Day Shift, Monday to Friday, 08:00 AM - 04:30 PM
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40 hours per week
Requirements:
- Speech Pathologist licensure in the state of practice: Required
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor: Required
- American Speech-Language and Hearing Association (ASHA) Certificate of Clinical Competence (CCC): Preferred
- Valid Driver's License and must be at least 21 years of age or older: Required
Roles & Responsibilities:
- Evaluate and diagnose speech, language, cognitive-communication, and swallowing disorders.
- Develop and implement treatment plans tailored to individual patient needs.
- Provide direct therapy services to patients in their homes.
- Collaborate with other healthcare professionals to ensure comprehensive care.
- Educate patients and families on treatment plans and strategies for improvement.
- Document and maintain accurate records of patient progress and treatment outcomes.
Compliance / Notes:
Mileage reimbursement is in addition to the standard rate.
