Job Title: Registered Nurse – Care Manager (Emergency Department)
Location: Bronx, NY
Type: Contract | 14 Weeks

Pay Rate: $55 - $60/hour

Position Overview:

Navitas Healthcare, LLC is seeking a Registered Nurse – Care Manager (Emergency Department) for an exciting Travel or Local job in Bronx, NY.

Responsibilities

• Coordinate patient care planning and discharge processes within the Emergency Department.
• Conduct utilization reviews to ensure appropriate level of care and resource utilization.
• Collaborate with physicians, nursing staff, social workers, and case management teams.
• Facilitate safe and timely patient discharge planning and post-acute care coordination.
• Maintain accurate and timely documentation in compliance with hospital standards.
• Support care transitions by identifying patient needs and arranging appropriate services.
• Participate in interdisciplinary rounds and care conferences.
• Ensure compliance with regulatory, clinical, and organizational policies.
• Utilize electronic medical records and computer systems for documentation and reporting.

Required Qualifications

• Minimum 5 years of clinical experience as a Registered Nurse.
• Strong experience in care coordination, utilization review, and discharge planning.
• Associate Degree in Nursing (ASN/ADN) from an accredited nursing program.
• Active New York State Registered Nurse License.
• BLS Certification.
• Computer literacy and proficiency with Electronic Medical Record systems.
• Primary Source Verification of RN License and Registration through NYSED.
• PRI and Screen Certification required within 3 months of employment.

Preferred Qualifications

• Bachelor’s Degree or Master’s Degree in Nursing.
• Leadership experience in a clinical or hospital setting.
• Case Management Certification.

Licensure & Certifications

• Active New York State RN License.
• BLS Certification.
• PRI and Screen Certification (within 3 months of hire).
• Primary Source Verification of RN License & Registration (NYSED).
• Case Management Certification (preferred).
