Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The role focuses on the creation of a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Key responsibilities include assessing patient needs and performing comprehensive assessments in accordance with RN scope of practice and hospital policies. The nurse considers the patient’s culture, age, and developmental level. Identifying patient risk factors and presenting complaints is essential. Collecting health information from families or others as necessary is also a part of the role. Objective and subjective data is collected and verified as needed. A problem list is developed while prioritizing patient needs based on input from the inter-professional team. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family based on the medical plan of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy within scope of practice. The nurse explains procedures, treatments, and processes to patients and their families. Actions are initiated to alleviate patient stress and discomfort. Recognizing, responding to, and communicating changes in patient conditions appropriately is crucial. Ongoing assessment and response to procedures and therapies are documented according to hospital policy. Tasks are delegated appropriately and team members' performance of patient care activities is overseen. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients are identified and addressed according to their level of comprehension. The patient discharge process is facilitated by executing discharge orders in a timely manner. Identifying potential discharges and assuring the performance of discharge teaching is essential. Active participation in unit-based activities supporting organizational values and priorities is expected. A positive group dynamic and healing environment are promoted through both words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practices, current research, and caring philosophies. Participation in department and unit quality improvement activities and core measures is encouraged. A holistic perspective regarding patient care is maintained throughout the role.
