Duties

Major duties include, but are not limited to:

• Provides and supervises nursing care using all steps of the nursing process.
• Assesses, plans, implements, and evaluates age-specific patient care.
• Coordinates care focused on patient education, self-management, and satisfaction.
• Ensures care delivery follows established policies, procedures, and guidelines.
• Collaborates with interdisciplinary team members to influence care outcomes.
• Actively participates in unit/program quality improvement and customer service initiatives.
• Manages patient care, coordination, and discharge/disposition planning for inpatient and outpatient settings.
• Assists Veterans, families, and caregivers in navigating complex health care needs and service options.
• Supports care for patients with acute, chronic, complex, catastrophic, or life-threatening conditions.
• Coordinates with multiple providers across all levels and sites of care.
• Addresses psychosocial, nursing, and medical needs through interdisciplinary care planning.
• Evaluates care processes and outcomes to ensure timely, appropriate services.
• Demonstrates leadership in improving holistic, evidence-based care.
• Provides peers with constructive feedback and fosters a supportive professional environment.
• Contributes professional nursing perspective in interdisciplinary discussions.
• Partners with colleagues to drive change and achieve optimal patient outcomes.
• Shares case management knowledge to support safe, quality nursing care.
• Shares educational findings and participates in building an evidence-based practice culture.
• Questions clinical practices to support continuous improvement and evidence-based care.

Pay: Competitive salary, regular salary increases, potential for performance awards
