Schedule: Monday–Friday | 8:30 AM – 5:00 PM
Weekly Hours: 37.5

Overview

We are seeking an experienced RN Care Delivery Manager to support high-quality, participant-centered care while overseeing daily clinical operations and interdisciplinary coordination. This role blends clinical leadership, care management, staff oversight, and regulatory compliance, with a strong focus on outcomes, satisfaction, and continuity of care for a frail and elderly population.

Key Responsibilities

• Develop, review, and oversee individualized care plans to improve outcomes and satisfaction

• Coordinate and participate in Interdisciplinary Team (IDT) meetings, SDRs, and care transitions

• Provide care management across settings including home care, rehab, nursing homes, and acute care

• Collaborate on UAS reviews, clinical reports, documentation accuracy, and quality audits

• Ensure timely investigation and documentation of incidents and grievances

• Balance staffing resources and provide clinical oversight across assigned sites

• Support onboarding, training, performance evaluations, and supervision of nursing staff

• Ensure compliance with organizational policies and state/federal regulations

• Act as Manager on Call during non-operational hours, as assigned

• Serve as Director of Patient Services (DPS) for LHCSA when designated

Must-Have Qualifications

• Active, unrestricted NY State RN license

• RN, BSN required or 5+ years of relevant healthcare experience

• Minimum 2+ years administrative/management experience in CHHA, LHCSA, hospital, SNF, or clinic

• Supervisory experience required

• Experience with frail or elderly populations

• Strong care coordination, documentation, and communication skills

• NYSED-approved Infection Control coursework completed

Nice to Have

• BSN preferred

• Case Management Certification (or willingness to obtain within 2 years)

• Managed Long-Term Care or PACE experience

• Bilingual skills

Additional Requirements

• Legally authorized to practice in NY State

• Up-to-date immunizations and medical clearance

• Ability to meet physical and cognitive demands of a clinical leadership role

This position requires acting within RN scope of practice and supporting continuous quality improvement across care delivery.
