Position: LPN Home Health

Type: Full-Time Contract

Duration: 8 weeks (high possibility of extension)

Shift: Days | Mon to Fri | 8 am to 5 pm

Guaranteed Hours: 40 per week | 8 per day

Facility: Signature Health Services Home Care

Location: 1300 Rollingbrook Dr Ste 306, Baytown, TX 77521

Tentative Start: 11/02/2026

Charting: Wellsky/OASIS

Requirements:

1 year of LPN Experience Required
6 months of Home Health Experience Required
LPN License TX Required
BLS Required
Driver's License Required
COVID Card or Exemptions allowed

Unit Notes:

- Region: East Houston (30/40 mile radius).

- Milage: GSA Max.

- Setting: Outpatient (some visits are in Assisted Living Facilities - roughly 20%).

- Traveler can work weekends/holidays if necessary for patient but will have to be approved by their supervisor.

- Traveler will be required to attend a daily conference call at 8:15AM.

- Scrubs: clean/professional with closed toe shoes - no facial jewelry except small nose stud and earrings.
