About the Opportunity

Our client is seeking two Family Nurse Practitioners to join an established outpatient primary care team near Canandaigua, New York..

This is an opportunity for an FNP who enjoys comprehensive primary care, continuity of care, patient education, and working collaboratively with physicians, advanced practice providers, nurses, and other members of the care team.

Clinical Responsibilities

• Provide comprehensive outpatient primary care across the lifespan
• Diagnose, treat, and manage acute and chronic conditions
• Perform preventive care and wellness visits
• Provide chronic disease management and patient education
• Develop and maintain long-term relationships with patients and families
• Collaborate with physicians, APPs, nursing staff, and interdisciplinary teams
• Maintain accurate clinical documentation in the electronic medical record
• Participate in quality improvement initiatives and support evidence-based care

Requirements

• Licensed Family Nurse Practitioner (FNP)
• Active New York RN license or eligibility for New York RN licensure
• At least 1 year of experience preferred in:

• Primary care
• Family medicine
• Internal medicine
• Community health
• Strong patient-centered approach
• Excellent communication and clinical decision-making skills
