If you're passionate about helping children and adolescents overcome challenges and build brighter futures, we invite you to become part of our mission-driven organization.As a Case Manager, you'll serve as a primary advocate for youth in our residential program by coordinating services, supporting treatment planning, collaborating with families and community partners, and helping ensure every child receives the care and resources they need to succeed.Working alongside therapists, nurses, residential staff, educators, and community agencies, you'll play a vital role in creating positive outcomes for youth through trauma-informed, relationship-based care.Every day is an opportunity to provide hope, stability, and a path toward healing.Why You'll Love Working Here Make a meaningful difference in the lives of children and adolescents Join a mission-driven nonprofit with over 100 years of service Work alongside an experienced multidisciplinary treatment team Build strong relationships with youth, families, and community partners Supportive leadership committed to employee development Comprehensive benefits package Paid time off and holidays Retirement plan Opportunities for continuing education and professional growth A workplace grounded in the Sanctuary Model of trauma-informed careWhat You'll DoAs a member of our Residential Treatment team, you will:Coordinate case management services for youth receiving residential treatment.Develop and maintain supportive relationships with youth and their families.Schedule, coordinate, and document Treatment Plan Meetings.Conduct regular individual meetings with assigned residents.Coordinate services with therapists, physicians, psychologists, schools, juvenile probation, child welfare agencies, and other community partners.Complete required assessments, including CAFAS, APS Care Connect, and other clinical documentation.Maintain timely, accurate case notes, monthly summaries, home visit documentation, court reports, and treatment records.Communicate regularly with the West Virginia Department of Health and Human Resources (DHHR) and other referral agencies.Attend multidisciplinary team meetings, court hearings, and MDT meetings as needed.Assist with discharge planning and securing appropriate placement options for youth.Monitor documentation for completeness and submit required billing documentation each month.Respond appropriately to crisis situations and communicate significant incidents to appropriate agencies and team members.Collaborate with the Clinical Team to review incidents and support ongoing treatment planning.Help maintain compliance with agency policies, licensing standards, and documentation requirements.Support Star of Hope's mission by promoting professionalism, teamwork, confidentiality, and respect in every interaction.Transport youth as needed in accordance with agency policies.What We're Looking ForRequired QualificationsBachelor's degree in Social Work, Psychology, Human Services, Counseling, or a closely related field.Valid driver's license with an acceptable driving record.Ability to successfully complete required background screening.Preferred QualificationsWest Virginia Social Work License (LSWA, LGSW, or higher).Experience in one or more of the following:Residential treatmentChild welfareBehavioral healthCase managementJuvenile justiceFoster careCommunity mental healthExperience working with trauma-informed care, multidisciplinary teams, and state child welfare systems is highly valued.The Ideal CandidateYou are someone who:Builds trusting relationships with youth and families.Communicates effectively with professionals, community partners, and team members.Stays organized while managing multiple priorities and deadlines.Maintains professionalism during crisis situations.Demonstrates sound judgment and strong documentation skills.Believes in trauma-informed, relationship-based care.Values accountability, integrity, compassion, and teamwork.Wants to make a lasting difference in the lives of children and adolescents.Why Star of Hope Family Services?For more than a century, Star of Hope Family Services has been helping children, youth, and families build brighter futures.
