Remote Care Manager for Children & Families
CLCF
CLCF, a care coordination nonprofit in Westchester, seeks a Care Manager to provide compassionate support, advocacy, and services to children and families facing medical, emotional, or behavioral challenges.
The role is a remote contract with monthly in‑person visits across New York, reporting to the Care Manager Supervisor. A bachelor’s or master’s degree with relevant experience is required, with a minimum five active cases and travel within NY.
