Family Nurse Practitioner

Metro Community Health Centers is seeking an experienced Family Nurse Practitioner to join its Bronx location following an unexpected resignation. This provider will join an established team consisting of a Medical Director (MD) and Physician Assistant and will inherit an existing patient need within a busy FQHC setting that focuses on working with individuals with developmental disabilities. Leadership strongly prefers someone who can step in with minimal training and practice independently.

Overview

• Team includes Medical Director (MD) and Physician Assistant
• Patient population primarily adults, though practice accepts patients ages 5
• Schedule Mon-Fri with no expectations past 5 pm and clinic closed on weekends
• Women's wellness visits performed in primary care; gynecologic cases can be referred to onsite/affiliated GYN provider that rotates through every other week
• Standard scheduling template:
  • 20-minute follow-up visits
  • 40-minute new patient/complex visits
• Shared call rotation: 1:7
• Existing leadership prefers an experienced practitioner who can operate independently
• Will consider less-experienced candidates if they have meaningful OPWDD/developmental disability experience as an RN or provider

Requirements

• Board Certified Family Nurse Practitioner (FNP) required
• Active New York license (or ability to obtain)
• Prior primary care experience strongly preferred
• OPWDD, FQHC, community health, Medicaid/Medicare, or underserved population experience preferred
• Experience caring for patients with intellectual/developmental disabilities highly desirable
• Ability to manage a primarily adult patient population while caring for ages 5
• Comfortable participating in a shared call schedule
• Strong preference for candidates requiring limited mentorship and onboarding support

Compensation & Interview Process

• Compensation target: $140,000-$160,000
• Current provider benchmarks:
  • DNP earning approximately $145,000
  • NP earning approximately $135,000
• Two-step interview process:
  1. Initial interview with leadership team
  2. Onsite interview with the Medical Director and Nellie (COO)
