General Description The purpose of this job is to coordinate services and support to individuals residing in an Intermediate Care Facility. The QIDP is responsible for the wellbeing of the ICF consumers and advocates for their rights. This role is in charge of and coordinating services to individuals enrolled in Home based Community Services (HCS) and Texas Home Living (TxHmL) and Intermediate Care Facilities (ICF). Ensures assistance in the provision of access to medical, social, educational, vocational, and other services to help individuals become independent. Create and conduct training to assist consumers in developing, maintaining or increasing their independence. This class works under general supervision, independently developing work methods and sequences. This role reports to the Program Director of IDD Community Services. Duties And Responsibilities Assists individuals and Legal Authorized Representative (LAR) in planning, coordinating, and monitoring services. Initiate process for home modifications when authorized and monitors to ensure compliance with HHSC and HCS Billing requirements. Process billing logs, invoices, and coordinate payments. Maintains compliance with all Joint Commission training requirements relevant to job duties, including but not limited to those dealing with use of restraints and physical holding of individuals receiving services. Ensures Individual Plan of Care (IPC), Implementation Plan (IP), and Intellectual Disability Related Conditions Assessments (IDRCA) are renewed and/or revised in accordance with HHSC timeframes. Works with supervisors to conduct quarterly reviews of SPO's and overall progress of ICF consumers. Communicates with LAR's and conducts annual IDT meetings to discuss continued stay of consumers on ICF and their overall progress in the unit. Develops specific program objectives and individual program plans. Manages consumer treatment and follows up with treatment plans. Advocates for consumers Performs other duties as assigned. Minimum Education And Experience Requirements Requires a Bachelor's Degree in a social, behavioral, or human service field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice supplemented by one (1) year of professional experience in an MH/IDD setting / behavioral health preferably with the IDD population or closely related field; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills and abilities. Experience with trauma-informed services; cognitive behavioral therapies, including DBT; and motivational therapies including the use of incentives, preferred. Required Knowledge And Abilities Knowledge of trauma-informed theories, principals, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred. Physical Demands Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations. Unavoidable Hazards (Work Environment) Involves routine and frequent exposure to: Bright/dim light; Dusts and pollen. Other extreme hazards not listed above. Special Certifications and Licenses Must be able to be credentialed as a Qualified Intellectual Disability Professional (QIDP) in the timeframe designated by EHN. Must possess and maintain a valid state Driver's License with an acceptable driving record. Must be able to pass a TB, criminal background and drug screen.
