Shift Type: Rotating shifts including weekends

Candidate Pay Rate: $55 per hour

Roles & Responsibilities

Coordinate patient care plans across multidisciplinary teams
Advocate for patients and families throughout their healthcare journey
Ensure smooth transitions between care settings
Collaborate with physicians and specialists to optimize outcomes
Document and track patient progress with attention to detail

Qualifications

Active RN license in Florida or compact state
Case Management experience preferred
Strong communication and organizational skills
Ability to work independently and as part of a team

Knowledge & Skills

Patient advocacy and care coordination
Discharge planning expertise
Electronic medical records proficiency
Knowledge of healthcare regulations and compliance
