Key Responsibilities

Coordinate care for assigned clients and develop goal-oriented care plans, including Start of Care and ongoing reassessments
Conduct onsite supervisory visits to evaluate clients, family environments, and caregivers, and complete timely documentation
Ensure availability, proper use, and maintenance of required equipment and supplies
Provide supervision, training, orientation, and ongoing education to clinical field staff
Deliver excellent customer service and maintain strong communication with clients and families
Provide nursing updates and obtain re-authorization for continued care
Develop ISP plans and quarterly reports for DD waiver clients using the WAMS system
Ensure compliance with DMAS regulations and agency policies (preferred knowledge)
Maintain professional nursing standards in all interactions and documentation
Perform additional duties as assigned, including caregiver and family training when required

Qualifications

Active Registered Nurse (RN) license required
CPR and First Aid certification required
COVID-19 vaccination required
Minimum of 2 years of home care experience
Willingness to travel within Fairfax, VA and surrounding counties
Strong understanding of home health care operations and compliance requirements

Preferred Qualifications

Experience with DD Waiver programs
Home health care experience
CCC+ Medicaid experience
Working knowledge of DMAS regulations and compliance standards
