In this role, you will provide counseling, support, and case management services to patients and their families. The ideal candidate will have strong clinical skills, excellent communication, and a passion for helping individuals navigate emotional, behavioral, and social challenges. Responsibilities: · Conduct psychosocial assessments and develop individualized care plans · Provide individual, group, and family counseling to patients · Collaborate with physicians, nurses, and interdisciplinary teams to support patient care · Connect patients and families with community resources and support services · Document patient progress and maintain accurate, confidential records · Support crisis intervention and provide guidance in high-stress situations · Advocate for patients' needs and assist in care planning and discharge coordination Requirements · Degree in Social Work (MSW) from an accredited program · Current state license as a Licensed Clinical Social Worker (LCSW) · 1+ year of clinical experience in a healthcare or mental health setting (preferred) · Strong counseling, assessment, and case management skills · Excellent communication and interpersonal abilities · BLS certification may be required depending on setting
