Responsibilities:

Review EMRs to determine eligibility and create orders.
Conduct video intake assessments and treatment plans.
Document, educate, and escalate cases as needed.

Qualifications:

Active RN or LPN with 3-5 years cardiac or chronic care.
Epic experience and strong remote communication skills.
