Occupational Therapist (OT) – Skilled Nursing

Sacramento, CA

We are seeking an Occupational Therapists (OT) to join supportive, patient-focused therapy team in a skilled nursing facility .

Responsibilities

Evaluate patients and develop individualized treatment plans
Provide skilled physical therapy services to help patients improve mobility, strength, balance, and independence
Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
Document patient progress and adjust treatment plans as appropriate
Provide patient and caregiver education
Support safe discharge planning and continuity of care

Qualifications

Current California Occuaptional Therapist license
Graduate of an accredited Occupational Therapy program
Strong communication and patient-care skills
Multiple Sacramento-Area Locations
