Services may include screening, evaluation, direct service, consultation and/or instruction.Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client’s age, condition, and needs.Implement client care plan activities based on treatment goals towards independent living for the client/family.What You’ll Bring:Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution.Current PT license in the State of Maine.Current CPR card required.1-year Physical Therapy experience preferred.
