Family Advocate
VNS Health
As a Family Advocate at VNS Health, you will support patients and caregivers across New York City by linking them to vital home and community-based services. Working within the Community & Social Services department, you’ll conduct family assessments, coordinate care with nurses and social workers, and help navigate healthcare and social support systems. You will provide education, advocacy, and crisis support while promoting culturally sensitive, patient-centered care.
