Nurse Practitioner

The Cancer Survivorship Program Nurse Practitioner or Physician Assistant provides comprehensive follow-up care for patients who have completed active cancer treatment. This role focuses on surveillance for recurrence and secondary malignancies, management of late and long-term treatment effects, health promotion, and coordination of care to support long-term wellness and quality of life for cancer survivors.

• Provide longitudinal survivorship care to patients following completion of cancer treatment

• Conduct comprehensive survivorship assessments, including physical, psychosocial, and functional evaluations

• Develop and implement individualized survivorship care plans in collaboration with oncology providers

• Monitor for cancer recurrence, secondary malignancies, and late effects of cancer treatment

• Manage treatment-related complications such as fatigue, pain, neuropathy, cognitive changes, sexual dysfunction, and cardiometabolic risk

• Provide preventive care and health maintenance consistent with survivorship guidelines

• Educate patients and families regarding survivorship issues, healthy lifestyle practices, and symptom management

• Coordinate care with medical oncology, radiation oncology, primary care, specialists, and supportive services

• Facilitate referrals to rehabilitation, behavioral health, nutrition, cardiology, fertility, or other specialty services as appropriate

• Participate in survivorship education, wellness programming, and patient outreach initiatives

• Maintain accurate and timely clinical documentation

• Participate in quality improvement, outcomes tracking, and survivorship program development

• Comply with all regulatory, accreditation, and professional standards

• Graduate of an accredited Nurse Practitioner or Physician Assistant program

• Current NP or PA certification and state licensure

• Eligibility for hospital or clinic credentialing and privileging

• DEA registration or eligibility

• Prior experience or specialized training in oncology, survivorship care, or chronic disease management preferred

• Strong clinical assessment and chronic disease management skills

• Knowledge of cancer treatment modalities and long-term survivorship issues

• Ability to manage complex, multisystem health concerns

• Excellent communication, counseling, and patient education skills

• Ability to work collaboratively within multidisciplinary oncology and primary care teams

• Commitment to evidence-based, patient-centered survivorship care

• Outpatient cancer survivorship clinics or cancer care institutes

• Predominantly weekday clinic schedule with minimal or no call depending on program structure

• Ability to sit, stand, and move throughout clinic environments

• Manual dexterity for clinical examinations and documentation

• Visual and auditory acuity sufficient for patient assessment and communication
