Per Diem Dietitian
Job Summary:
Ingraham Manor Rehab and Nursing, a leading provider of rehabilitation and skilled nursing care in Bristol, CT, is seeking a per diem Dietitian to join our team. As a Per Diem Dietitian, you will play a vital role in ensuring the nutritional needs of our patients are met and exceeded. Responsibilities:
Plan and implement individualized nutrition care plans in collaboration with the interdisciplinary team
Conduct dietary assessments and develop personalized meal plans for patients with various medical conditions
Monitor and evaluate patient progress, making adjustments to nutrition care plans as needed
Provide nutrition education and counseling to patients, families, and healthcare staff
Develop and maintain accurate and up-to-date records of patient care and nutrition plans
Collaborate with the food service team to ensure menu plans meet patient needs and dietary restrictions
Stay current with industry developments, trends, and research to maintain expertise and knowledge
Requirements:
Bachelor's degree in Nutrition
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Familiarity with electronic medical records (EMRs) and nutrition care software
