Wedgewood Garden Care Center

Freehold, NJ 07728
Provide individual and group support to residents and their families.
Conduct psychosocial assessments and develop care plans.
Collaborate with interdisciplinary teams to ensure holistic care.
Maintain accurate and timely electronic records.
Advocate for residents’ rights and access to community resources.
Active Licensed Clinical Social Worker (LCSW) New Jersey.
Compassion and a genuine interest in working with the senior population.
Strong organizational and communication skills.
Medicare/Medicaid credentialing is a plus or willingness to obtain credentials.
Ready to hit the ground running and make a difference!
