In-Home Social Work Case Manager (Redding)

Home & Health Care Management in Redding, CA, seeks a Social Worker (MSW/BSW) to join the Care Management Team. You will collaborate with a nurse and SW to provide comprehensive in-home case management for older adults, children, and individuals with disabilities. You bring a Master’s or Bachelor’s in Social Work, strong problem‑solving skills, and computer literacy; experience with EMR is preferred. The role is full‑time at our Redding office, with 8am–5pm weekdays and no weekends. #J-18808-Ljbffr
