Schedule Monday-Friday Full-Time Day shift only Key Responsibilities Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed); Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists; Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment; Develops and maintains a plan of care that describes interventions to attain expected outcomes; Implements and records interventions to attain expected outcomes in EMR; Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections; Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.); Maintains a positive and respectful attitude to all patients and colleagues; Occasional travel to other nearby locations; and Medication administration and documentation. Requirements Valid RN license 6 months of RN experience strongly preferred 3 months of hemodialysis experience Valid drivers license and transportation Ability to travel covering multiple locations BLS certification (American Red Cross or American Heart Association Required); Ability to lift up to 35 pounds is required This is a safety sensitive position Depending on your job status, you may be eligible for the following benefits: 401(k) package Flexible Spending Accounts Education Assistance Certification & Professional Membership Reimbursement Compensation Salary Range: $40.00 - $60.00 per hour. *Based on experience.* Compensation offers are based on education, experience, skills, and geographic region. #J-18808-Ljbffr
