Dialysis Social Worker

U.S. Renal Care

U.S. Renal Care seeks a Dialysis Kidney Care Options Educator to guide patients and families through understanding kidney disease and dialysis choices. In this patient-first role, you will deliver individualized and group education on in-center and home therapies, support shared decision-making, and coordinate closely with nephrologists, nurses, social workers, and dietitians. You will assess learning needs, simplify complex information, and document education plans while upholding clinical excellence and regulatory standards.
