Provides safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care./ppShifts Available:/pulliFull Time, 32 hours a week, Days/Shifts Vary as Needed - 3:00pm - 11:30pm/li/ulpExpected starting wage range for LPN is $23.64 - $29.22 (hourly). Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements./ppDepending on what shifts you are asked to work: Shift differentials for hourly staff (6% for PM Shift, 10% for Overnight Shift). Weekend Shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift)./ppEssential Functions/pulliDemonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders./liliEstablishes and maintains a therapeutic relationship with members served./liliProvides nursing care and assists in the welcoming, admission, and discharge process./liliCollects data relevant to members served care; develops and implements nursing care plans./liliDemonstrates knowledge of crisis management and intervention./liliTranscribes physician orders accurately and in a timely manner./liliAdministers medication and treatments to members served./liliReceives and presents change of shift reports as assigned./liliAdheres to medication management policies and procedures./liliNotifies the prescriber and nursing supervisor of any changes in behavior or condition./liliDocuments the progress of members served toward their Recovery Plan goals./liliAttends and participates in community meetings and groups./liliActively participates in multidisciplinary team meetings and treatment planning meetings./liliCoordinates and participates in counting medications and giving report at the beginning and end of each shift./liliMust attend CPI training. Must assist with de-escalation as needed./li/ulpWhat You Bring to the Table (Must Have)/pulliGraduation from an accredited LPN program and current LPN licensure in state of operation./liliCurrent CPR/BLS certificate./liliMust be at least 18 years of age./liliMust be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment./liliAll opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification.
