Kids For The Future

Kids for the Future is seeking an RN Care Manager for their Hamilton County operations. You will provide evidence-based Care Transitions Interventions and manage older adults' transitions from hospital to home. Responsibilities include bedside visits, follow-up health coaching, and ensuring a safer recovery for patients. The role emphasizes clinical judgment, independence, and building meaningful relationships.
