Dependable Hospice & Palliative Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Tucson hospice team on a per diem basis .

This is an excellent opportunity for an experienced LPN who wants flexibility while providing meaningful, patient-centered care to individuals and families during one of life’s most important stages.

What You’ll Do

• Provide skilled hospice nursing care to patients in their homes or residential settings

• Perform nursing visits according to the established plan of care

• Monitor patients for changes in condition, symptoms, comfort, and overall well-being

• Administer medications and treatments as ordered and within LPN scope of practice

• Assist with pain and symptom management under the direction of the RN

• Educate and support patients, families, and caregivers

• Communicate changes in patient condition promptly to the RN Case Manager and interdisciplinary team

• Complete accurate and timely clinical documentation

• Collaborate with the hospice team to ensure continuity and quality of care

• Maintain compliance with hospice policies, procedures, and applicable regulations

What We’re Looking For

• Current Arizona LPN license in good standing

• Current CPR/BLS certification

• Previous hospice, home health, skilled nursing, or related clinical experience preferred

• Strong assessment, communication, and documentation skills

• Compassionate and professional approach to end-of-life care

• Reliable transportation and ability to travel throughout the Tucson service area

• Ability to work independently while remaining connected to the interdisciplinary hospice team
