The Patient Care Manager serves as a key leader within the organization, fostering a collaborative environment focused on excellence, safety, and compassionate care.
Key Responsibilities
- Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements.
- Coordinate, plan, and monitor patient care services to achieve optimal clinical outcomes.
- Supervise and support clinical staff, ensuring consistent and appropriate delivery of home health services.
- Maintain effective communication with patients, caregivers, referral partners, field clinicians, and office personnel.
- Support quality improvement initiatives and departmental performance goals.
- Promote professionalism, accountability, and service excellence throughout the organization.
- Ensure compliance with Medicare guidelines and applicable federal, state, and local regulations.
- Maintain a safe work environment and reinforce safety protocols.
- Mentor and develop staff through coaching, education, and ongoing professional development opportunities.
- Current Registered Nurse (RN) license in the state of Texas.
- Previous home health experience required.
- Two or more years of management or supervisory experience preferred.
- Strong knowledge of Medicare Conditions of Participation and home health regulations.
- Excellent leadership, communication, organizational, and problem-solving skills.
- Ability to effectively manage multiple priorities in a fast-paced healthcare environment.
