Job Summary Full-time Physical Therapist – PERM/PT. In‑Office in York, PA, United States. 151‑bed SNF. Rate: $38–$44 per hour. Shift: Monday–Friday 8:00 am – 4:30 pm (1 shift). Start date: 05/31/26. Class: P‑PT. Area: Kingston Court Skil. Perm Placement Opening. Placement fee: 40 hours Mon‑Fri, week‑end rotation available. Responsibilities Provide patient‑centered physical therapy assessments and develop individualized treatment plans. Deliver evidence‑based PT interventions to restore function and reduce pain. Document progress and maintain accurate, timely records in EHR. Collaborate with multidisciplinary team to coordinate care and referrals. Set and track performance targets using pipelines to monitor progress. Maintain compliance with regulatory standards, safety, and infection control. Educate patients and families on exercises, home programs, and self‑management. Participate in professional development initiatives and pursue advancement opportunities. Demonstrate initiative to improve clinic processes and patient experience. Assist in scheduling and ensure on‑time patient flow to maximize throughput. Participate in on‑site audits and quality improvement projects. Experience / Qualifications PT licensure in the operating jurisdiction with an active state license. Bachelor’s or Doctorate in Physical Therapy from an accredited program. Ability to develop and monitor individualized treatment plans and outcomes. Minimum 1–2 years of outpatient PT with manual therapy and gait training. Proficiency with EHR documentation and standardized patient progress notes. Strong teamwork for coordinating multidisciplinary referrals and care plans. Knowledge of infection control, safety regulations, and regulatory compliance. Comfort with targets, pipelines, and performance metrics to drive throughput. Effective patient and family education for home programs and self‑management.
