Physical Therapist - Travel Contract

Bachelor's, Master's or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Florida PT license required to start the assignment. BLS certification from AHA or ARC may be required. Home health experience is required. Experience with HCHB electronic medical record preferred. Productivity experience or ability to meet productivity targets. Client productivity requirement: 6 points per day / 30 points per week. Ability to maintain weekly mileage at or under roughly 100 miles per week is preferred. Ability to collaborate in a small, team-oriented office environment. Orientation will be provided. Preferred: experience working with PTAs and OTs in outpatient or home health settings.

Assignment Details Facility type: Home Health. Shift: Days. Schedule options: 5x8 (Monday-Friday) or 4x10s. EMR: HCHB. Productivity expectation: 6 points per day / 30 points per week. Mileage guidance: aim to stay under 100 miles per week. Team composition: current staff includes multiple full-time PTs, PTAs, and OT staff; small office, team oriented.
